Patient Surveys Cross Road uses a third party company to conduct our patient satisfaction surveys. The company sends out surveys to a random sample of patients that have been seen during regular clinic hours. The surveys are designed to cover every aspect of your visit. Your responses to these surveys are combined and categorized so we can see which areas of our practice are most in need of improvement. There is also a space for written comments. Your comments are extremely valuable to us and are used to provide feedback to our staff on specific issues. If you receive a survey please take the time to fill it in completely and return it in the postage-paid envelope. We encourage parents and guardians to fill out surveys for minors under their care. |
Suggestion Box We have feedback forms and a suggestion box in our Clinic waiting room. We encourage you to take the time to fill in one of these forms whenever you visit. |
Correspond with Us Please use our Patient Comment and Privacy Complaint form and return to: Cross Road Medical Center |

We're Committed to Continuous Quality Improvement
Since 2004, Cross Road has used a formal system to collect, assess and respond to feedback from our patients. Our Continuous Quality Improvement (CQI) Committee meets monthly to discuss patient feedback. We then plan and implement improvements to our medical care delivery. The CQI Committee is currently headed up by Sherri Cox, RN, Director of Nursing.
All of the thoughtful feedback and constructive criticism we receive is taken seriously and handled confidentially.
